Tele-Health Assessments

A mental health assessment gives your therapist a picture of how you think, feel, reason, and remember. The mental health test assesses your emotional well-being via a series of questions and includes a review of your health history and health records.

We offer a variety of mental health assessments tailored to the client. For the most part a mental health assessment is designed to: diagnose mental health conditions such as anxiety, depression, schizophrenia, postnatal depression, eating disorders and psychotic illnesses differentiate between mental and physical health problems assess a person referred because of problems at school, work or home.

FMLA Evaluations & required forms 

The Family and Medical Leave Act (FMLA) provides job-protected leave to address mental health conditions. 



Pre-Bariatric Evaluations

We are licensed mental health professionals who regularly conduct pre-bariatric surgery mental health evaluations that meet surgeon requirements.

Emotional Support Animal Evaluations

An emotional support animal (ESA), or simply "support animal", is a companion animal that a medical/mental health professional has determined to provide benefit for an individual with a disability.

The purposes of the relationship is to assure that the supervisee’s clients receive appropriate professional service, to assure the supervisee’s appropriate professional development, & to otherwise fulfill the requirements for supervision of MH Interns.

Brief Therapy

A short - term therapy which focuses on setting goals and working out how to achieve them. The SFBT program consist of six sessions in total.
